Referral criteria
- Who we support
- Adults 18+ with learning disabilities, autism, mental health needs, physical disabilities or age-related frailty.
- Area covered
- Solihull, the wider West Midlands, Northampton, Milton Keynes, Herefordshire and Scotland.
- Package sizes
- From short visiting calls to 24-hour and shared supported living arrangements.
- Funding routes
- Local authority, NHS Continuing Healthcare, direct payments and private.
What to send us
- Current needs assessment or care and support plan
- Risk assessment and any positive behaviour support plan
- Medication list and health background
- Housing situation and tenancy status
- Proposed hours, start date and funding authority
